Take Charge of Your Diabetes Workshop

Thursdays starting Feb. 7 through March 14, 5 p.m. to 7 p.m., in HCMC Classrooms 2 & 3 Diabetes is a complicated disease that can strike fear, confusion, and helplessness in diagnosed people and caregivers. People with diabetes must deal not only with their disease, but also with the impact this has on their lives and emotions. A self-management approach to diabetes education gives people the knowledge, tools and confidence to take day to day responsibility of their diabetes care. The Paris and Henry County Healthcare Foundation and Henry County Medical Center will be offering a free Take Charge of Your Diabetes Program workshop.   The 6-week program is designed for people with diabetes and caregivers to learn basic skills necessary to self-manage their diabetes and work effectively with their healthcare professionals. The Take Charge of Your Diabetes Program is led by educators certified by Stanford University Diabetes Self-Management Program Master Trainers.  Classes will be held every Thursday from 5 p.m. to 7 p.m. Spaces are limited for the 2-hour workship.  The workshop is offered by the Paris and Henry County Healthcare Foundation Delta Rural Health Initiative. Wednesday, February 20, 2019 HCMC Classrooms 2 & 3 7 a.m. to 11 a.m. Our Chronic Disease Management Program offers screening including total lipid panel, blood sugar, blood pressure, BMI  and A1c.  if a person has a chronic condition , smokes or is overweight, they can join the program for free. Participants are screened every three months and receive education about their chronic conditions.   Anyone can show up the day of the screening to join the program.  All patients must be fasting for 8 hours to participate in the screening. Cardiac Rehab Nutrition Class

Ever wondered if you are selecting the best food choices for your heart?

Henry County Medical Center’s Cardiac Rehab as well as Food & Nutrition Services is offering you the opportunity to learn what to eat for a healthy heart thanks to the Paris-Henry County Healthcare Foundation. The class is free and you receive a FREE Nutrition Book to take home.  The class is limited to 24 participants and Cardiac Rehab patients have first priority to attend the class.  Please, we ask you leave children at home.  The cost for this class is free. Blood Pressure Checks – Henry County Office on Aging

HCMC Community Educator and/or Paris and Henry County Healthcare Foundation Chronic Disease Nurse Case Manager will be available to take your blood pressure and discuss the results with you on the first and third Tuesday mornings of each month (schedule could be modified due to holidays). Carroll County Office on Aging Chronic Disease Management Screening

Wednesday, April 3, 2019 Carroll County Office on Agining ~ 625 High Street, Huntingdon, TN 9 a.m. to 11:30 a.m. HCMC and the Paris and Henry County Healthcare Foundation will be on hand at the Carroll County Office on Aging to offer a free chronic disease management screening.  Our Chronic Disease Management Program screening offers  total lipid panel, blood sugar, blood pressure, BMI  and A1c. There will be free information on our services as well as give-aways and more. 1 in 3 Americans has pre-diabetes and most do not know it. YOU MAY HAVE PREDIABETES and be at risk for type 2 diabetes if you:
  • Are 45 years of age or older
  • Are overweight
  • Have a family history of type 2 diabetes Are physically active fewer than 3 times per week
  • Ever had diabetes while pregnant (gestational diabetes) or gave birth to a baby that weighed more than 9 pounds
Join Us for an Informational Session about the Program! Monday, April 8, 2019 4 p.m. - 5 p.m. HCMC Private Dining Room Join us for the PreventT2 Diabetes Prevention Program and learn how to prevent this chronic health condition.  Participants who attend the informational session will receive a free A1c screening at the event. As part of a PreventT2 group, you will work with a trained lifestyle coach and other participants to learn the skills you need to make lasting changes. You will learn to eat healthy, add physical activity to your life, manage stress, stay motivated, and solve problems that can get in the way of healthy changes. PreventT2 is a year-long program with weekly meetings for the first 6 months, then meetings once or twice a month for the second 6 months to help you maintain your healthy lifestyle changes. Better Breathers Club

Better Breathers Clubs are welcoming support groups for individuals with COPD, pulmonary fibrosis and lung cancer, and their caregivers.  Learn better ways to better cope with lung disease while getting the support of others in similar situations. HCMC and the Paris and Henry County Healthcare Foundation will be providing trained facilitators to give you the tools you need to live the best quality of life you can.   The HCMC Better Breathers Club will begin meeting every 3rd Monday of each month in the HCMC Cafeteria from 2 p.m. to 3 p.m. and feature educational presentations on a wide range of relevant topics, including:
  • How COPD affects the lungs
  • Breathing techniques
  • Exercise
  • Talking with your physician
  • Medications and other treatment options
  • Medical tests
  • Supplemental oxygen
  • Home healthcare
  • Lung transplants
  • Air pollution
