9 a.m. to 10 a.m.

HCMC and the Paris and Henry County Healthcare Foundation will be on hand at the Henry County Office on Aging to offer a free chronic disease management screening.  Our Chronic Disease Management Program screening offers  total lipid panel, blood sugar, blood pressure, BMI  and A1c. There will be free information on our services as well as give-aways and more.

Better Breathers Club

Better Breathers Clubs are welcoming support groups for individuals with COPD, pulmonary fibrosis and lung cancer, and their caregivers.  Learn better ways to better cope with lung disease while getting the support of others in similar situations. HCMC and the Paris and Henry County Healthcare Foundation will be providing trained facilitators to give you the tools you need to live the best quality of life you can.   The HCMC Better Breathers Club will begin meeting every 3rd Monday of each month in the HCMC Cafeteria from 2 p.m. to 3 p.m. and feature educational presentations on a wide range of relevant topics, including:

  • How COPD affects the lungs
  • Breathing techniques
  • Exercise
  • Talking with your physician
  • Medications and other treatment options
  • Medical tests
  • Supplemental oxygen
  • Home healthcare
  • Lung transplants
  • Air pollution

Our Chronic Disease Management Program offers screening including total lipid panel, blood sugar, blood pressure, BMI  and A1c.  if a person has a chronic condition , smokes or is overweight, they can join the program for free. Participants are screened every three months and receive education about their chronic conditions.   Anyone can show up the day of the screening to join the program.  All patients must be fasting for 8 hours to participate in the screening.

Take Charge of Your Diabetes

Thursdays starting March 19 – April 23, 12 Noon to 2 p.m. in HCMC Classrooms 2 & 3

Diabetes is a complicated disease that can strike fear, confusion, and helplessness in diagnosed people and caregivers. People with diabetes must deal not only with their disease, but also with the impact this has on their lives and emotions. A self-management approach to diabetes education gives people the knowledge, tools and confidence to take day to day responsibility of their diabetes care.

The Paris and Henry County Healthcare Foundation and Henry County Medical Center will be offering a free Take Charge of Your Diabetes Program workshop. The 6-week program is designed for people with diabetes and caregivers to learn basic skills necessary to self-manage their diabetes and work effectively with their healthcare professionals. The Take Charge of Your Diabetes Program is led by educators certified by Stanford University Diabetes Self-Management Program Master Trainers. Classes will be held every Thursday from 5 p.m. to 7 p.m.

A free screening will be offered at the completion of the last class.

Spaces are limited for the 2-hour workship. The workshop is offered by the Paris and Henry County Healthcare Foundation Delta Rural Health Initiative.

Cost is free and open to anyone with diabetes. Spouses, family member and caregivers are welcome to attend.

Henry County Office on Aging Talk – Chronic Disease Management

The Paris and Henry County Healthcare Foundation and GrowWell West TN will be at the Henry County Office on Aging to discuss the Chronic Disease Management program they offer.  Join Tina Umstead, LPN, as she discusses how to manage chronic conditions and the services she offers through the program.  The event is free and open to all seniors.
